Researchers have integrated heart cells grown from human embryonic stem cells into a host heart

“This is an important proof of concept that heart cells can have the ability to integrate within existing cells,” says lead researcher Dr. Lior Gepstein of the Technion Faculty of Medicine.


Dr. Gepstein and his colleagues were the first to generate human heart tissue in the laboratory, which could be used for testing and creating new drugs for genetic studies, for tissue engineering and for studying the effects of various stresses on the heart. The team, however, wanted to test if the cells could function and integrate within a live host body.


They placed beating human heart cells, derived from the stem cell line, onto rat heart tissue. By recording the electrical activity from both tissues, they were able to show that the two tissues integrated electrically to form a single functional unit in which rat and human cells were beating in perfect synchronicity.


This concept was then tested in living animals with slow heart rates. The team transplanted human pacemaker cells derived from the embryonic stem cell line into the animals’ hearts. Using specialized electrical mapping equipment developed by the Technion researchers, they were able to pinpoint where the heart rate originated within the living animal, which turned out to be at the immediate region containing the transplant. This demonstrated that the implanted cells had integrated into the host heart, and by regulating its pace were behaving like a biological pacemaker.


Technion scientists are widely recognized as pioneers in stem cell research. Dr. Joseph Itskovitz-Eldor was among the team that first discovered in 1998 the potential of stem cells to form any kind of tissue. Technion researchers have since grown heart tissue, blood vessels, and human insulin-secreting cells from embryonic stem cells. And scientists at the Technion are now able to grow an infinite number of cells without a feeder layer, making these cells appropriate for eventual human transplantation.


Heart disease is the leading cause of death in the U.S. Abnormally slow heart rate develops in older patients, who can only survive with an electronic pacemaker. During heart attacks, tissue is destroyed when blood is temporarily cut off to a section of the heart, and this tissue can never be repaired. Cell therapy has the potential to treat these currently nearly untreatable diseases.


Dr. Gepstein says that while obstacles remain before the research leads to cell therapy, these groundbreaking results demonstrate that this type of treatment will be possible.

It’s one of the biggest controversies in fetal surgery and the cause of heated debate among surgeons and maternal-fetal drug physicians almost the in every respect: What’s the best withdraw to treat twin-twin transfusion syndrome (TTTS), one of the most common conditions requiring fetal surgery and the supreme cause of mortality in twins?

Dr. Crombleholme issued a clarion call to fetal care specialists around the world. “The earlier in gestation TTTS is diagnosed, the more severe the syndrome tends to be,” says Dr. Crombleholme, who is also a pediatric surgeon at Cincinnati Children’s Hospital Medical Center. “We have to shift attention to diagnosing this earlier in the progression of the syndrome and being more aggressive about intervention. We can’t conclude that one therapy is better than another, but it’s apparent that we’re going to compromise survival if treatment isn’t initiated before it gets to be severe.”


In TTTS, the way in which blood flows through the connecting blood vessels on a shared placenta is unequal. As a result, the donor twin has slowed growth and decreased amniotic fluid, while the recipient has too much amniotic fluid and develops a form of heart failure. Without treatment, TTTS is fatal for at least 90 percent of twins, and both twins are at risk for severe complications.


There are two main treatment modalities: amnioreduction, in which excess amniotic fluid is removed from the recipient twin, or a fetoscopic laser procedure to treat the anatomic basis for the syndrome. Until now, the only major study comparing these two treatments for TTTS was the Eurofetus trial, which compared laser and amnioreduction as primary therapies. That study, published in The New England Journal of Medicine in 2004, found that, at six months, there was better survival among twins using laser vs. amnioreduction.

The NIH-funded study, which began in 2002, required participants to have more severe disease and to have already failed an initial amnioreduction. But when the Eurofetus results were published, everyone thought that laser was superior and stopped referring patients. The American investigators asked that the trial be stopped. Unbeknownst to investigators at that time, the trial oversight committee found significant differences between the two treatment modalities and recommended that the trial be stopped, which it was in May of 2005.


The trial had uncovered higher mortality among recipient twins who received laser treatment. But overall, survival of one or both twins of the same pregnancy was no different between the two treatments. In addition, there was no difference between survival rates of donor twins and recipient twins. How is this possible?


“It turns out that in amnioreduction, there were significant numbers of treatment failures, so patients were dropping out in the amnioreduction arm because they were failing therapy, whereas they were dying in the laser therapy,” says Dr. Crombleholme. Statistically, these patients are considered to have had the same outcome: a treatment failure. So, ultimately, there was no significant difference in survival of one or both twins between the two treatments.”


What Dr. Crombleholme and his colleagues did find were two predictors of success. One was the stage of severity of TTTS. The other was the echocardiographic findings of heart failure. Fetal echocardiography, which is not done routinely in TTTS in most centers, should be done whenever a patient is diagnosed, according to Dr. Crombleholme


“While one cannot conclude from this study that one therapy is better than the other, neither can one conclude that they’re equivalent, because it included patients who failed an initial amnioreduction and then went on to get laser,” says Dr. Crombleholme.

The researchers then followed up with the participants five years after leaving school to check their height and weight.


The researchers found that increased participation in physical education and certain extracurricular physical activities decreased the likelihood of being overweight as an adult. The likelihood of being an overweight adult was most reduced among teens who participated in wheel-related extracurricular activities, such as rollerblading, biking or skate-boarding more than 4 times per week. These teens were more than twice as likely to maintain a normal weight as adults compared to their less active peers. However, no impact was detected when physical activities were performed fewer than three times per week.


The American Academy of Pediatrics and the U.S. Department of Health and Human Services recommend physical education at all grade levels. Studies show that less than half of high school students are enrolled in physical education courses. Only 6 percent of junior high schools and 5 percent of senior high schools offer daily physical education, according to the Institute of Medicine.


“Sixteen percent of adolescents in the United States are overweight or obese and 85 percent of obese teens will become obese adults. School-based physical education could be a low-cost strategy and a long-lasting solution to adult obesity,� said Blum.

ACOG Joins Opposition To Mandatory HPV Vaccine Requirement For Immigrant Girls And Women

The American College of Obstetricians and Gynecologists (ACOG) joins more than 100 organizations representing immigrants’ rights, women’s rights, public health, medicine, and reproductive justice in urging the Centers for Complaint Direction and Inhibiting (CDC) to remove the woman papillomavirus (HPV) vaccine from the list of required vaccinations for female immigrants seeking undying residence or coming to the US. In the current study, Joakim Dillner, M.D. of Lund University in Malmö, Sweden, and colleagues compared the efficacy of 11 another screening strategies that cast-off HPV DNA testing, cytology, or a combination of the two. To evaluate the different strategies the investigators retrospectively analyzed matter from 6,257 women who were enrolled in the intervention arm of a large-hearted randomized screening fling, called Swedescreen, in which HPV DNA testing was hand-me-down in above moreover to standard cytology.

As seen in above-named trials, the use of HPV DNA testing in conjunction with cytology increased the screening efficacy, detecting 35 percent more cases of CIN grade 3 or worse, compared with cytology alone. The strategy, in what way, doubled the tally of tests required. By contrast, the use of the HPV primary screen with cytology and repeat HPV testing resulted in a 30 percent higher detection rate compared to cytology alone, with only a 12 percent increase in the number of tests required. In this approach, all women with HPV infections detected on the primary screening went on to have a standard Pap test; those with normal cytology then had a repeat HPV DNA test at least 1 year later.

“In conclusion, our data evince that using HPV DNA testing as peak screening followed by cytological triage and retell HPV DNA testing of women with normal cytology who are HPV DNA pigheaded after at least 1 year is a viable design for incorpo¬rating HPV testing in primary cervical screening because it improves supersensitivity and maintains a high [positive predictive value], thus minimizing unnecessary referrals,” the authors write.
